What we collect

When you send a message, we collect your name, your email, your phone number and the message you write, with how and when you would like us to reach you.

Booking

When you choose Book online, you leave this site for our scheduling page, which has its own terms.

What we do with it

We use it to reply to you and to book a visit. We do not sell it, and we do not share it except as the law allows.

Your records

We keep your records private and share them only as the law allows. Please do not put medical details in a message to us; we ask for them on the intake form and at your visit. We send your doctor's office a note only with your written okay.

Your choices

Call or write to us to see what we hold, to correct it or to ask us to remove your message.
